Firstly, you will receive an email from us containing your Annual Fee Disclosure Statement (FDS) and Ongoing Consent form. The first section of this document provides an overview of the services provided and the total fees paid during the last 12 months. By signing this form via Adobe Sign, you are agreeing to continue to receive advice and ongoing service and agree to continue paying an ongoing fee for these services for the next 12 months. Email Two For clients who pay their fees via their superannuation or investment fund, you will receive an Ongoing Adviser Service Fee form to be signed via Adobe Sign. This form will be specific to your superannuation or investment fund and will renew your ongoing fees with Evergreen for the next 12 months.
